As life expectancy in the United States continues to increase, the growing aging population is expected to experience a higher risk of chronic diseases. About 60% of older adults, 65 and older, are currently living with 2 or more chronic conditions. These long-term illnesses e.g. Alzheimer’s disease, cancer, hypertension, cardiovascular disease, diabetes etc., and their complications, are known to contribute to a reduction in quality of life, and an increased risk of mortality among those affected.

It is estimated that there are about 2 million older adults suffering from a serious or debilitating long-term condition that renders them homebound. Among this population, some are sick and frail, bedridden or are otherwise physically unable to travel to their healthcare providers to receive the required care. Another 5 million people among this aging population are wheelchair-bound, or have difficulty leaving their home without help from a caregiver. Consequently, proper access to care is critical, but can also prove to be severely challenging among those who need it most.

We at Winslow Primary Care Alliance know that the emotional and physical strain on people living with chronic conditions is a burden that is shared among family members and other caregivers. These stresses often lead to negative health consequences for those trying to cope with the demands, sometimes even causing the caregivers themselves to require additional services in order to maintain their own wellbeing.

The role of home-based primary care medical services has become increasingly relevant in today’s day. Similar to medical house calls in previous times, in-home physician visits today can help to address and mitigate this burden of care for the aging population and their caregivers, while helping to improve health outcomes for people living with chronic conditions. While convenience is a huge benefit of in-home primary care visits, as importantly, our in-home providers spend more time with patients, and are able to assess whether other factors within the home may be limiting or negatively affecting their wellbeing. Our doctors are then able to make informed suggestions for improvement, and include these recommendations in the patients’ comprehensive care plans.
