How do I enroll to receive home-based primary medical care by Winslow Primary Care Alliance?
There are several ways to initiate primary care continuity services in your home with Winslow Primary Care Alliance. You may contact us by calling our business office at (703) 896-7711.
To request physician home visit services online, either send us an email at [email protected] or send us your request online through the contact us feature on our website.
You will receive a response to your inquiry within 48 hour- at which time we will gather general health information as an initial pre-screening. An initial assessment home visit Is required. After the initial assessment, you will receive verbal or written confirmation If there is mutual agreement that Winslow Primary Care Alliance will become your primary care practice providing continuity care for you or your loved one.
You may download our intake forms and either fax or email them to us prior to your appointment. It is most efficient to complete the intake forms prior to your first assessment so that more time is spent evaluating clinical matters rather than on paperwork and other clerical tasks. The first initial assessment generally takes 1 1/2 to 2 hours.
May I keep my existing primary care doctor and still receive home-based primary care services with Winslow Primary Care Alliance?
We do not provide transitional care at-this-time. In-home primary medical services through our practice is offered only to clients who agree to allow us to become their primary care practice. We require your permission to request medical records from your office-based primary care doctor and specialists for safe and efficient transfer of care.
How much does a home-based primary care visit cost?
We accept most major health insurances which cover in-home primary care services. There are many benefits in receiving in-home primary medical care through our practice. You will have access to your doctor 24 /7, will overcome difficulties in leaving your home, nor wait in an office waiting room for an hour or more to see a primary care doctor. Therefore, convenience fees may be applied. Please contact our office for further information.
Which medical insurance carriers do you accept?
Contact our office for an insurance eligibility review. During the intake process, we will run an insurance eligibility check to determine if your insurance covers home-based primary medical care services.
Considering the benefit of having a doctor come to your home, having 24/7 direct access to your physician and not having to experience uncomfortable and prolonged periods waiting in a clinic waiting room with physical limitations, to see a doctor, convenience fees may be applied.
Can I receive home-based primary care with your practice if I do not have medical insurance?
Yes. We offer fee-for-service payment options. Please contact our office to discuss payment and fee schedules.
How do I schedule a home-based Primary care visit?
You may contact our office or schedule an appointment using our online scheduling tool.
For urgent health needs as they arise, please contact your doctor directly or contact our administrative office at 703 896-7711. Depending on the circumstance, you should receive an in-home medical visit by your doctor if necessary, within 24 hours or less.
If the urgent circumstance involves chest pain, difficulty breathing, signs and symptoms of a stroke which include: slurred speech, weakness in one part of the body, deviation of the facial muscles, or sudden inability to walk, please do not hesitate to call 911.
May I schedule a physician home visit on the weekends?
Weekend home visits are provided on a case-by-case basis depending on the clinical circumstance and the availability of your doctor. Please contact your doctor directly for urgent health care needs for further information and guidance.
How can I contact my in-home primary care doctor?
Once you have agreed to receive in-home primary care continuity services with Winslow Primary Care Alliance, your primary care doctor will give you their cell phone number so that you may call and talk directly with your doctor for urgent and immediate needs.
For non- urgent questions or to request medication refills, you will receive access to our online patient portal. During business hours, you may contact our administrative office to leave a message and your doctor will return your call within 24 hours for non-urgent matters. Requests for medication refills are completed within 48 hours.
What if I am hospitalized?
If your doctor recommends emergency room care or if you are hospitalized for matters that cannot efficiently be addressed in your home, your primary care doctor may visit you in the hospital, communicate with the hospital inpatient medical team and provide a post hospitalization home visit within 24-48 hours of discharge depending on the medical circumstances for which hospitalization was required.
Does your practice provide IV infusion treatments or IV antibiotics?
Your primary care doctor may arrange IV antibiotics or other IV infusions in your home where possible. If long-term IV access is needed additional out-of-pocket fees may be required. Long-term peripheral IV access may be inserted by a local interventional radiology unit. Transportation costs for such service may require out-of-pocket costs if not covered by insurance.
Laboratory work may be collected during your in-home primary care visit or by your skilled nursing agency upon receiving a lab order request from your doctor.
Does your practice offer transitional care?
We provide short-term or transitional care services on a case-by-case basis.
For hospital personnel who have identified patients in need of transitional or short-term home-based primary care services, we are happy to consider new clients.
