Winslow primary care is one of the few family medicine practices that offers in-home primary care services for those who experience physical, social or mental limitations preventing them from seeking care at an outpatient clinic.

To increase access to quality primary care services for elderly and individuals with limited mobility, we offer quality primary care right within the homes of those who need it.

We enjoy listening and value each of our clients as unique. We care about the total health care needs of individuals we serve, want to know about daily challenges, and strive to maximize quality of life with respect to the medical conditions our clients may experience.

During a home visit, a physician will have the chance to directly observe and understand the home environment, nutrition and how personal sources of support might impact the overall health of our patients. We consider each of these factors in determining a primary care plan that is unique the individual receiving care.

Using the art of medicine, along with applying clinical science, an experienced doctor will manage your health conditions and offer wellness strategies in the comfort of your home to the extent that can be safely provided outside of a medical facility.

As your home-based primary care practice, our doctors may provide preventive health care and manage your chronic medical conditions that are amenable to home based care within the scope of primary care. We may arrange imaging, lab work, and other testing procedures to be completed in your home and when the need arises, provide referrals to specialty care by assisting in arrangements of non-emergency transportation services that offer stretcher and wheelchair support.

Working in collaboration with allied health teams, we also communicate with specialists and physicians who have been involved in prior treatments and planning so that there are fewer communication barriers and prompt attention to health changes thereby reducing unnecessary visits to the emergency room and preventable hospitalizations.
